Patients whose family origin lies in the Indian subcontinent have an extremely high mortality due to coronary heart disease, and their CHD often occurs in conjunction with the metabolic syndrome, noted Dr. Hobbs, professor of primary care and general practice at the University of Birmingham (England).</p>

<p>  He reported on 160 hypertensive British ethnic Asian Indians in a double blind, 36-week clinical trial at 17 primary care centers in the United Kingdom. They were randomized to antihypertensive therapy with the alpha-1 receptor blocker doxazosin or bendroflumethiazide.<br />
  At 21 weeks and 34 weeks, patients on doxazosin had significant reductions in total cholesterol, triglycerides, and serum glucose levels following a standard 2-hour oral glucose tolerance test compared with baseline. The bendroflumethiazide group had increases in total cholesterol, triglycerides, and serum glucose at both time points.<br />
  Fasting insulin was significantly lower at 21 and 34 weeks in the doxazosin group than in bendroflumethiazide group. Levels of nonesterified fatty acid particles were significantly lower at 30 minutes and at 2 hours in patients on doxazosin compared with bendroflumethiazide-treated patients at 21 weeks but not at 34 weeks.<br />
  Both agents lowered blood pressure to a comparable degree. Amlodipine was required to reach the target diastolic blood pressure of less than 90 mm Hg in 15.4% of the doxazosin group and 22% of patients in the bendroflumethiazide group.<br />

			<content:encoded><![CDATA[<p>	In older men, the inner part of the prostate around the urethra may continue to grow, a condition called benign prostatic hyperplasia (BPH) - also referred to as an enlarged prostate or swollen prostate.  Perhaps a fourth of men have some degree of h<span id="more-42"></span>yperplasia by the time they reach their fifties.  By the eighth decade, over 90% of males will have prostatic hyperplasia  (according to the American Urological Association).</p>
<p>However, in only a small number of cases (about 10%) will this hyperplasia be symptomatic and severe enough to require any surgical or medical therapy.  The mechanism for hyperplasia may be related to an accumulation of dihydrotestosterone in the prostate, which then binds to nuclear hormone receptors which then trigger the growth. The normal prostate weighs 20 to 30 gm, but most prostates with nodular hyperplasia can weigh as much as 50 to 100 gm.  And nodular hyperplasia is NOT a precursor to cancer.  A mildly increased PSA (4 to 10 ng/mL) in a patient with a very large prostate can be due to either nodular hyperplasia, or to prostatitis, rather than cancer.  </p>
<p>It&#8217;s good to know that BPH (enlarged prostate) is a non-cancerous enlargement of the prostate that is very common in men as they age. The inner part of the prostate around the urethra may continue to grow.  While this can cause problems with urinating because it can press on the urethra (the tube that carries the urine), BPH is not cancer.</p>
<p>The class of drugs usually used to conventionally treat BPH are the alpha 1-adrenoreceptors, including prazosin, alfuzosin, indoramin, terazosin, doxazosin, and tamsulosin. </p>
<p>Both BPH and prostate cancer, however, can cause similar problems in older men.  The &#8220;free PSA test&#8221; may help determine the difference between BPH and prostate cancer.  Prostatitis, BPH, prostate cancer and metastasis are all very common prostate ailments.  Many of these symptoms, however, can also be caused by other conditions, such as a urinary tract infection (UTI), acute prostatitis, or even a sexually transmitted disease.</p>
<p>Doctors must determine if a patient&#8217;s symptoms are due to prostate cancer, BPH, or to a number of other non-cancer-related conditions.  Your doctor conducts a digital rectal exam in order to diagnose BPH.  Treatment choices for BPH also include an ever watchful waiting.</p>
<p>Also regular checkups are important even for men who have had surgery for BPH.  Learn all you can about the treatment options for these prostate disorders if you&#8217;re having prostate symptoms that are keeping you up at night or keeping you up at night with worry. <p>  Your new patient is a 57-year-old African American man. His blood pressure is 150/95 mm Hg, fasting glucose 115 mg/dL, body mass index 32, and triglycerides 155 mg/dL; he is on no prior medications. During the course of his care you diagnose hypertension with metabolic syndrome and decide to recommend an antihypertensive. Thiazidetype diuretics are your standard initial therapy, but this patient has metabolic syndrome, and you know that certain antihypertensive agents have a more favorable metabolic profile than thiazide diuretics. Furthermore, metabolic differences among races have been touted as reason to use other agents in black patients. Should you recommend a thiazide diuretic, or another agent?<br />
  Until now, we&#8217;ve had no simple approach to treating hypertension in patients with metabolic syndrome&#8211;and half or more of our hypertensive patients over the age of 55 have this disorder.<br />
  Now, however, we can base decisions on clinical outcomes data from a subgroup analysis of the Antihypertensive and Lipid-Lowering Treatment to Prevent Heart Attack Trial (ALLHAT). (1) This new subgroup analysis supports use of thiazide-type diuretics in these patients&#8211;particularly in black patients&#8211;despite the more favorable metabolic profile of calcium channel blockers, alpha-blockers, and angiotensin-converting enzyme (ACE) inhibitors.<br />
  Cost is no longer as big a factor as it once was, now that ACE inhibitors and alpha-blockers, as well as thiazide diuretics, are available generically.<br />
  * Does a better metabolic profile improve outcomes?<br />
  We have had reason to be concerned about the metabolic adverse effects of thiazide-type diuretics in the past. Studies published before this ALLHAT subgroup analysis showed that hydrochlorothiazide for essential hypertension had adverse effects on potassium, glucose, and lipid metabolism. Some speculated that these changes aggravate the metabolic changes in early diabetes (2-4) and may contribute to increased coronary heart disease risk. (5,6)<br />
  ACE inhibitors and ARBs<br />
  The metabolic benefits of ACE inhibitors and angiotensin-receptor blockers (ARBs) are widely known. In comparative studies prior to this ALLHAT subgroup analysis, ACE inhibitors were thought to be preferable to thiazide diuretics and beta-blockers for patients with obesity (7) or the metabolic syndrome. (8) These agents also protect against diabetic nephropathy. (9) Other studies attribute additional vascular benefits to ACE inhibitors, (10) beyond that of lowering blood pressure.<br />
  A 2005 meta-analysis by Abuissa et al (11) showed that ACE inhibitors and ARBs were associated with significant reductions in the incidence of newly diagnosed diabetes, which, in turn, might lead to reduced heart disease. That report concluded that use of ACE inhibitors or ARBs should be considered in patients with prediabetic conditions such as metabolic syndrome, hypertension, impaired fasting glucose, family history of diabetes, obesity, congestive heart failure, or coronary heart disease.<br />
  Calcium-channel blockers and alpha-blockers<br />
  Calcium-channel blockers and alpha-blockers also do not appear to have the adverse metabolic effects of thiazides, and have also been advocated over beta-blockers and thiazides for hypertensive patients with metabolic syndrome. (12-15)<br />
  Racial differences<br />
  In a consensus statement developed before the findings from the ALLHAT subgroup analysis were available, it was noted that racial differences in metabolic syndrome may make the selection of antihypertensive agents particularly important in African American patients. (16)<br />
  * ALLHAT and JNC7 recommendations<br />
  The 2002 ALLHAT demonstrated that chlorthalidone (a thiazide-type diuretic) is superior to lisinopril, amlodipine, and doxazosin in preventing 1 or more major forms of cardiovascular disease. No difference was observed, however, for fatal coronary heart disease, nonfatal myocardial infarction, or all-cause mortality. (17) These findings persisted in subgroup analyses stratified by race, diabetic status, and level of renal function, but ALLHAT did not identify patients with metabolic syndrome a priori.<br />
  The ALLHAT influenced the 2003 Joint National Commission VII (JNC7) Report, which recommends thiazide diuretics for first-line treatment of hypertension in the absence of compelling indications to begin an alternative antihypertensive agent. (18)<br />
  Special consideration, but no recommendation. The JNC7 Report mentions the metabolic syndrome as a special consideration, but does not explicitly recommend a first-line therapy other than thiazides.</p>

The prostate gland is about the size of a walnut and is found at the base of the bladder. A thin tube that allows the passage of urine out of the penis (urethra) runs through the prostate gland. Fluid produced by this gland helps to nourish and support sperm, which come from the seminal vesicles via the ejaculatory ducts into the urethra.</p>
<p>Prostate cancer is a disease <span id="more-39"></span>in which the cells of the prostate become abnormal. They start to grow uncontrollably, forming tumors. A tumor is a mass or lump of tissue made of abnormal cells. Tumors may be malignant or benign. A malignant tumor can spread to other parts of the body. Malignant tumors are cancerous. Benign tumors cannot spread to other parts of the body.</p>
<p>Causes</p>
<p>Increasing age and the presence of normal male hormones (testosterone and dihydrotestosterone [DHT]) are the best-known factors that contribute to BPH. A hormone known as 5?-reductase converts testosterone into DHT in the prostate gland, which results in prostate enlargement. DHT concentrations in BPH tissue have been shown to be increased when compared with normal prostate tissue.</p>
<p>The actual cause of prostate enlargement is unknown. It is believed that factors linked to aging and the testicles themselves may play a role in the growth of the gland. Men who have had their testicles removed at a young age (for example, as a result of testicular cancer) do not develop BPH.</p>
<p>For some men, it suddenly becomes impossible to urinate (known as acute retention). Studies have shown that acute retention affects between 1 and 2 per cent of men with BPH each year. This condition is very painful and demands immediate medical treatment to avoid damage to the kidneys, among other things.</p>
<p>Treatment</p>
<p>The surgeon (urologist) must first prove that the prostatic enlargement is the cause of the symptoms. He may find it helpful to measure the urinary flow rate. This is a simple test: you pass urine into a large funnel and the time taken for a particular volume of urine to pass is calculated electronically. The urologist will also arrange an x-ray or ultrasound scan to make a picture of the bladder after an attempt at emptying. This will reveal whether the prostate is causing obstruction to the urethra. The ultrasound scan will also check that the kidneys are normal. In some hospitals, the size and the shape of the prostate can also be measured by ultrasound.</p>
<p>Medications</p>
<p>Alpha blockers. These drugs were originally developed to treat high blood pressure. They relax muscles around your bladder neck and make it easier to urinate. Four alpha blockers have been approved by the Food and Drug Administration (FDA) for treatment of BPH: terazosin (Hytrin), doxazosin (Cardura), tamsulosin (Flomax) and alfuzosin (Uroxatral).</p>
<p>Remedies</p>
<p>Pumpkin seeds are an effective home remedy for prostate problems and many patients have been helped by their use. These seeds are rich in unsaturated fatty acids which are essential to the health of the prostate. Persons beyond the age of fifty, who have any signs of prostate trouble; or those who suffer from urinary disorders of some kind, namely, burning or difficulty in passing urine freely, should take sixty to ninety grams of pumpkin steeds per day.</p>

1. Separate conditions but one causes the other<br />
It may start of as a headache, but the pain brings on dizziness as well. Or you may well experience dizziness or vertigo (spinning type of dizziness) and it ends up triggering a headache or making an existing o<span id="more-38"></span>ne worse. Even  though considered to be separate conditions, there can be a causal link. Even in terms of treatment, successfully treating of one symptom can improve the other.<br />
2. It&#8217;s a migraine<br />
Dizziness can be part of a migraine attack. They are known as migraine associated vertigo. The dizzy spells may happen before, during or after the headache.<br />
As with basilar migraine, dizziness or vertigo are part of its aura symptoms along with partial vision loss, double vision, severe vomiting, ringing in the ears (tinnitus), slurred speech, loss of coordination, numbness (on one or both sides of the body), weakness, and general confusion. These symptoms generally subside with the onset of the actual headache, though it is also possible that they can last for several days after the pain goes away.<br />
Basilar migraine is very rare, but potentially very dangerous as it can lead to strokes or transient ischemic attack (TIA).<br />
Hemiplegic migraine is also a rare condition linked to a genetic abnormality. Some of its symptoms are temporary motor paralysis and sensory disturbances on one side of the body preceding the actual headache, often accompanied by numbness or a pins-and-needles sensation. Also vertigo, double vision, hearing problems, difficulty in speaking and swallowing due to numbness around the mouth. The symptoms may cause a person to think they are having a stroke. However, unlike a stroke, the effects are reversible.<br />
It is possible that during a migraine attack, just experiencing the pain, nausea and discomfort may make you feel dizzy. Dizziness could also happen after a migraine attack, like a hangover.  Those  with migraine associated vertigo also often suffer from motion sickness.<br />
3. They are symptoms of certain medical problems<br />
A headache coupled with dizziness could be symptoms of a medical problem you are having.<br />
Below are some medical problems associated with them:<br />
-Head injury<br />
-Neck trauma<br />
-Temporomandibular Joint Syndrome (TMJ): Other symptoms include pain every time you talk, chew, or yawn.  Possible ear pain, ringing in the ears (tinnitus) and hearing loss  Also clicking and popping of the jaw.<br />
-Heat exhaustion: Besides headache and dizziness, person will look pale, sweat profusely, have clammy skin, feel weak, thirsty, nausea and vomit.<br />
-Stroke: The symptoms of stroke are very sudden. You will experience numbness or weakness of the face, arm or leg, especially on one side of the body, confusion, trouble speaking or understanding,  vision problems in one or both eyes, trouble walking, dizziness, loss of balance or coordination, severe headache with no known cause.<br />
4. They are side effects of certain medications<br />
Many medicines have side effects. The following medications have headache and dizziness included in their list of common side effects.<br />
-Antihypertensive drugs or drugs that lower blood pressure such as cardizem (diltiazem hydrochloride), accupil (quinapril hydrochloride), cardene (nicardipine hydrochloride), and cardura (doxazosin mesylate).<br />
-NSAIDs. For example, Ibuprofen have listed rash, ringing in the ears, headaches, dizziness, drowsiness, abdominal pain, nausea, diarrhea, constipation, and heartburn as possible side effects.<br />
-Antiangina drugs are also known as nitrates. These drugs relieve symptoms of angina which is severe pain in the chest caused by heart disease. Before the body is able to adjust to the medication, you may experience headache. If the headache continues on for a few weeks or get worse, check with your doctor.  Other common side effects are dizziness, light headedness, increased pulse, nausea, vomiting, a flushed face and neck and restlessness.<br />
The list of medications above are definitely not conclusive. There are others that have headache and dizziness listed as common side effects. Therefore, know the medications you are taking and consult with your doctor if you have any concerns.<br />
5. Substance exposure<br />
Carbon monoxide poisoning: It is called the silent killer as the gas is invisible, odorless and colorless. Symptoms of carbon monoxide poisoning are headaches, dizziness, nausea, light headedness and shortness of breath.<br />
Inhalent abuse:  Toluene can be found in gasoline, paint thinners and removers and correction fluid. Exposure to high doses of toluene can result in headache, euphoria and delirium, giddy feelings, uncoordinated  movements, confusion, nausea and vomiting.</p>
